As a reminder: rebate submissions open November 1st, 2026 and will be accepted up until December 15th, 2026. We can’t wait to see everything you’ve been working on!
Rebate Submission Acknowledgment
By proceeding with this submission, I acknowledge and agree to the following:
- My rebate checklist includes all active social media accounts I would like reviewed.
- My rebate report is fully completed and includes all required supporting documentation.
- My submission is being processed through the distributor with whom I am currently enrolled in the Devoted Creations Five Star Program.
- I understand that missing or additional documentation submitted after my initial submission will be reviewed on a case-by-case basis and may result in my rebate being moved to the end of the processing queue.
- I understand that incomplete submissions may delay processing or prevent my rebate from being approved.
- If I have any questions regarding my submission, I will contact rebates@devotedcreations.com before submitting.
Terms and Conditions: This Incentive Form must be submitted by the undersigned salon owner to qualify for the Devoted Creations, Inc. Annual Incentive Program. The qualifying period is for purchases occurring between November 1st, and October 31st. The undersigned attests that all of the information provided and invoices attached are true and accurate. The undersigned acknowledges that Devoted Creations, Inc. retains the right to 1. Authenticate all information contained on this form, 2. Qualify the salon purchases with its above listed distributor, 3. Verify compliance with this program. 4. This is an incentive for the salon to continue to purchase Devoted Creation’s products payment requires the salon to be operating as a salon business at the time of payment, with the same ownership, and 5. 5. ALL INFORMATION MUST BE PROVIDED ALL INFORMATION MUST BE PROVIDED DIRECTLY FROM A SINGLE AUTHORIZED PARTICIPATING DEVOTED CREATIONS DISTRIBUTOR.DIRECTLY FROM A SINGLE AUTHORIZED PARTICIPATING DEVOTED CREATIONS DISTRIBUTOR. The undersigned verifies that this is the only request filed for the qualifying period. If any information contained in this request is false or inaccurate, Devoted Creations may void this incentive offer. All submissions must occur between November 1 and December 15. Salon acknowledges that this offer is void where prohibited by law, and represents that this request is lawful.
